Uniform Order Form Information

Please complete this uniform order form to order your players Uniforms and Trainings Kits. 

As described in the registration, uniforms are not included in your players registration and is an additional cost. It is your responsibility to ensure that the correct uniforms are ordered for your player.

Please read the following for uniform order information and guide to know what uniforms you will need to order for your player based on the program their registered for.

Home Uniform Kit ----- $70

Away Uniform Kit ------ $70

Training Kit -------------- $50

Uniform Purchase Guide

Aussies players order ----------- 1 Training kit only.

Futures Players order ----------- 1 Training kit only.

Academy Players order --------- 1 Home Uniform Kit, 1 Away Uniform Kit, 1 Training Kit.

Select Players order ------------- 1 Home Uniform Kit, 1 Away Uniform Kit, 1 Training Kit.

Important Uniform Information ​

• Uniforms are only updated/changed yearly.

• All ASC families are required to fill out this order form by March 26, 2023, payment is due by March 26, 2023. If no full payment is received your uniform order will not be processed.

• Payment plans are unable to be made for uniform fees. 

• Parents with two players or more will need to submit an order form for every player.

• Turnaround time for uniforms is within 2-3 weeks.  

• After completion of the uniform order form, you will receive a uniform invoice sent out to the primary email, we have on file associated with your player(s) registration.

• Parents have the option to buy more than one of the same uniform kits for their players if they wish to do so.
